New Jersey Immigration Medical Exam and Form I-693

Adjustment-of-status applicants who live in New Jersey may be required to submit Form I-693. The immigration medical examination must be completed by a doctor currently designated by USCIS as a civil surgeon. The designation—not the doctor’s ordinary state license alone—authorizes completion of the immigration form.

Verify the Doctor Before the Appointment

Search the current USCIS civil surgeon directory. Confirm the address, designation, fee, laboratory arrangements, vaccine charges, and documents required directly with the office.

Bring Complete Records

Bring identification, available vaccination history, and medical records relevant to the examination. Missing records can lead to repeat testing or additional vaccines. Never buy a signed form from someone who does not perform the required examination.

Submission and Timing

USCIS rules governing Form I-693 timing and validity can change. Follow the current form instructions and the newest USCIS notice in the individual case. Keep the physician’s envelope sealed and retain a copy of the completed form.

Consular Processing

A person applying for an immigrant visa abroad follows the embassy’s instructions and uses an approved panel physician. A New Jersey civil surgeon generally is not the correct provider for that process.

Call 203-373-9080 for a brief telephone evaluation or use the form on this page. A detailed attorney consultation, by secure video or confirmed office appointment, is ordinarily paid. Contacting the firm does not create an attorney-client relationship.
